All the medical information concerning your stay in our establishment is recorded in your file.
You are entitled to have direct access or you may duly appoint a doctor to have access to the information contained in your medical file.
You must send your request in writing to the management, enclosing a copy of your identity card, in order to obtain the information contained in your file; you can also consult your file on our premises; we can also send you photocopies at your expense.
The information thus requested can only be made available to your after a minimum period of forty-eight hours following your request and should, as far as is possible, be conveyed to you within 8 days at the latest. However, if the information dates back over five years, this period is extended to two months.
Your medical file is kept in a suitable and safe place for a minimum period of twenty years commencing the date of your last stay or consultation.

When you are admitted to the Center, you have the possibility of appointing, in writing, a trusted person whom you choose from among your entourage. This person will be consulted in the event that you are unable to express your wishes. This appointment can be revoked at any time.
If you so wish, your healthcare proxy can accompany you throughout the various stages of your healthcare and attend medical meetings in order to help you make your decisions.

Any adult person can, if they so wish, draw up advance healthcare directives, sometimes referred to as a living will, in the event that, near the end of their life, they are unable to express their wishes.
These directives indicate the patient’s wishes with regards to limiting or discontinuing treatments. They may be revoked at any time. The doctor takes the patient’s wishes into account when making decisions about diagnostic procedures, medical operations and treatments on condition that the directives have been drawn up at least three years prior to the patient’s state of unconsciousness. If you would like your directives to be taken into account, remember to make them available to the doctor in charge of your healthcare at the Center.

Personal information concerning patients is detailed in the Center’s computer files. It is processed as part of the management of hospital stays and the economic analysis of medical activities.
The data concerning you is subject to automatic processing under the terms and conditions laid down by the Act of 6th January 1978. This data is conveyed to the doctor in charge of the Center’s medical information department by the doctor managing the unit in which you have received treatments or by the doctor who compiled your file: this data is protected by doctor-patient confidentiality and medical secrecy.
You are entitled to oppose, on legitimate grounds, the collection and processing of your personal data, according to the conditions laid down in Article 38 of the Act of 6th January 1978. Therefore, your right of opposition may only be exercised when the processing of personal data being called into question does not meet a legal obligation.
You are also entitled to have access and to amend the information conveyed to the doctor in charge of the information department by the practitioner who compiled the file.
A patient experience questionnaire will be handed to you just before you leave the Center. We hope that you will fill it out in order to help us improve the quality of patient care and service – thank you for your participation.

All the staff at the Center are on hand to listen to you. If you would like to make any suggestions or remarks about your experience, we invite you to speak directly to the manager in charge of the department or the consultant handling your file. Nevertheless, if you are not entirely satisfied, you can write to the Center’s Director (30, Rue de Paris – 92100 Boulogne-Billancourt).
The Director will ensure that your observations, complaint or remarks are dealt with in accordance with the provisions stipulated in the French Code of Public Health (R.1112-91 to R.1112-94). He will send your letter to the Commission for Patient Advocacy and Treatment Experience Quality (CRUQPC). He may, if needs be, put you in contact with a mediator, doctor or non-doctor, who is a member of the CRUQPC.
The mediator(s) will meet you, and possibly your family, in order to examine the difficulties you are facing.
CRUQPC is tasked with ensuring that your rights are respected and with helping you in the processes you have to undertake. It recommends measures to be adopted to improve the welcome and care given to hospitalized people and their relatives.
When drafting these recommendations, CRUQPC particularly refers to your observations, suggestions, remarks and complaints.
